2.3. Sufficient contact information for the copyright owner or authorized representative, such as a physical address, email address, and telephone number.
2.4. A statement that the copyright owner has a good faith belief that the usage of the material is not authorized by the copyright owner, its agent, or the law.
2.5. A statement that the information in the notification is accurate and, under penalty of perjury, that the complaining party is authorized to act on behalf of the copyright owner.
2.6. The copyright owner's or its authorised representative's electronic or physical signature.
If you believe the material you posted on DomainCheckTools was removed or access was disabled in error. You have the right to use that material, and you may file a counter-notification. The counter-notification should include the following:
4.1. Identification of the material that has been removed or to which access has been disabled.
4.2. Your name, address, phone number, email address, and a statement that you consent to the jurisdiction of the federal district court for the judicial district in which your address is located (or the federal district courts located if your address is outside of the United States) and that you will accept service of process from the person who filed the DMCA Notice or their agent.
4.3. A statement, under penalty of perjury, that you have a good faith belief that the material was removed or disabled due to a mistake or misidentification.
4.4. An electronic or physical signature.
